2010 – 2011 Preschool Registration Form
Please note:
- Families of children currently enrolled in our programs will have priority acceptance provided the registration form and non-refundable registration fee of $45.00 are received on or before April 1, 2010. Checks should be made payable to Effort Baptist Church.
- Registration forms and fees will only be accepted in the church office.
- Please bring an official (vital records) copy of your child’s birth certificate so we may photocopy it. Unofficial copies of birth certificates will not be accepted.
- Our office hours are Monday through Friday 9:00 a.m. – 4:30 p.m. On March 18, from 6:00 p.m. – 7:30 p.m., the church office will be open to receive registration forms, fees, and to photocopy birth certificates.
